29 CFR 1910.134(e)(4)(i): The medical questionnaire and examinations were not administered confidentially during the employee's normal working hours or at a time and place convenient to the employee: a) Geisinger Medical Center- On or about October 26, 2023, the employer maintained employees' completed respirator medical questionnaires. Abatement certification required. ABATEMENT NOTE: In order to maintain strict confidentiality of the information obtained in the questionnaire, the employer's role is limited to distributing the blank questionnaire to the employee for him or her to fill out or providing it to the PLHCP, who will administer the questionnaire to the employee. If the employer provides the questionnaire to the employee, an addressed and postage-paid envelope should also be provided for the employee to mail it to the PLHCP.
