29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a) Construction Site, 12711 Sunset Dr, Firestone, CO 80504: On or about December 05, 2023, employees were exposed to fall hazards of approximately 20 feet to the ground below installing gutter on the second level of an existing residential home without fall protection.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   a) Construction Site, 12711 Sunset Dr, Firestone, CO 80504: On or about December 04, 2023, employees were exposed to fall hazards while using a self-supporting ladder in a manner it was not designed for. The six-foot fiberglass step ladder was a self-supporting ladder being used as a non-self-supporting ladder to install rain gutter on the second level of a residential home.
