29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   Worksite located at 9809 Russell Circle, Liberty, Missouri- Employees were exposed to fall hazards while using ladders to access the roof were not raised at least three feet above the upper landing surfaces.
