29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.   a. On the roof of a temporary residential worksite, located at 5058 Lawnview Street, Jacksonville, FL 32205: On or about December 7, 2023, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using nail guns to install wood and underlayment paper.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a.  On a 5:12 pitched roof of a residential worksite,  located at 5058 Lawnview Street, Jacksonville, FL 32205:  On or about December 7, 2023, the employer exposed six employees to fall hazards ranging from 10 to 16 feet, in that, fall protection was not utilized by the employees while performing roofing activities.
