29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): A.) Superior Forge and Steel, Inc. DBA Superior Forge and Steel Corporation, on or about 11/30/23 and times prior thereto: An employee engaged in grate removal activities of the pit did not utilize fall protection systems to protect against the 12 foot fall hazard. Employee was at edge hooking up grating and employee fell into the pit. Grate removal activities include but are not limited to, putting alloy steel chains on the grating. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

DBA Superior Forge and Steel Corporation, on or about 11/30/2323 and times prior thereto: lockout did not occur prior to employees conducting clean up and chip hopper removal operations associated with the pit area of the Waldrich Roughing Lathe. Cleaning operations include but are not limited to hooking up grates, lifting hoppers out of the pit, and sweeping the pit area. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
