29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition:  a)  1s floor Leasing Center - On or about December 11, 2023, employees were exposed to slip, trip, and fall hazards where the carpet was ripped and there were electrical cords in the path of egress.   b) Upstairs Administration - On or about December 11, 2023, employees were exposed to slip and tripping hazards where the carpet was ripped.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred:  a) On or about January 5, 2024, the employer did not record a work-related injury on their CY 2024 OSHA 300 log and OSHA form 301, for a Maintenance Technician, who tripped and fell while going down the stairs leading to the boiler room #4 causing head and neck injuries.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
