29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: (a) Vehicle storage area; The protective plastic at the female plug-end of a 12 AWG flexible extension cord, connected to a corded circular saw, was broken with a piece of the plastic missing, exposing energized inner conductors.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Vehicle storage area, precast room; The outer insulating jacket of a 12 AWG flexible extension cord, connected to a portable heater, was damaged and open at the base of the strain relief boot, exposing inner wires and rendering the strain relief ineffective as pull was transmitted to the terminal screws.
