29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  (a) On or about December 13, 2023, the employer did not certify that each operator had been trained and evaluated as required. The employer did not have certification for warehouse employees who operated forklifts.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.40(a): Basic requirement. When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.  (a) JBS Logistics & Warehousing , worksite at 2043 Corporate Lane, IL: on or about December 13, 2023, the employer failed to provide within four business hours the requested OSHA 300 Log of Work-Related Injuries and Illnesses for 2023 year-to-date, and OSHA 301 Injury and Illness Incident Report(s) for all recordable injuries in 2023 year-to-date.  There is no abatement certification or documentation required for this item.
