29 CFR 1926.501(b)(13): Workers engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented.  On or about December 11, 2023, workers were performing framing activities on the fourth floor of an apartment under new construction without the use of a fall protection system, exposing workers to a fall hazard of approximately 40 feet.

On or about December 7, 2023, employees were picking up debris around a covered trash chute under construction without the cover being secured, exposing employees to a fall hazard of approximately 12 feet to the lower level.
