29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: 309 Newington Road, West Hartford, Connecticut roofing project: On or about December 15, 2023, and at times prior thereto, the employer did not ensure that head protective equipment was used when employees were exposed to impact hazards of falling material and/or tools when working in the fall zone of the roof eave from an approximate height of 8 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: 309 Newington Road, West Hartford, Connecticut roofing project: On or about December 15, 2023, and at times prior thereto, employees were using an extension ladder to access the roof as the siderails of the ladder used to access the roof only extended approximately .5 feet (6 in.) above the upper landing surface exposing them to a fall hazard of approximately 20 feet.
