29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold frame was used as a means of access:  a) 3727 Baring St Philadelphia, PA 19104 - On or about December 13, 2023, the employer did not ensure that a means of access was provided when scaffold platforms were more than 2 feet above a point of access.  Scaffold frame was used as a means of access. Employees were required to perform masonry work from a scaffold without ladder access, thereby exposing the employee to falls up to approximately 28 feet from the ground.

An employee was required to perform masonry work from a scaffold platform without guardrails or other means of fall protection, thereby exposing the employee to falls approximately 28 feet from the ground.  BIS Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vi) which was contained in OSHA inspection number 1615439, citation number 1, item number 2 and was affirmed as a final order on March 9, 2023, with respect to a workplace located at 2026 N 6th St, Philadelphia, PA.
