29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)  Top of the house being built - On or about December 15, 2023, the employer instructed his employee to help him frame the roof rafters by walking on top of them without any fall protection exposing the employee to fall hazards.

29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  (a) North side of the house - On or about December 15, 2023, the employer built a wooden box that was open to the front, attached to the forklift forks, and used to raise the employees 14 feet high to gain access to the house exposing employees to fall hazards.
