29 CFR  1926.1053(b)(1): Ladder extension was as such that ladder deflection under a load, did by itself, cause the ladder to slip off its support.  Location: a) Exterior back wall of the Building.  On or about 12/15/2023, employees were exposed to a fall hazard as they were utilizing an extension ladder that was deflected while an employee was on the ladder. The fall was approximately 20 feet from the ladder to the ground below.
