29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  (a)  On or about December 11, 2023, at 9545 Palm Isles Dr., Boynton Beach Fl 33437, the employer did not ensure personal fall arrest system was used while an employee performing demolition work of a skylight was exposed to a 24-foot fall hazard through holes (including skylights) to ground level.
