29 CFR  1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, and the ladder was not secured at its top to a rigid.  (a) On or about Dec 14, 2023, #3A Canaan Scherpenjewel GNS St., St Thomas,  VI  00802, employees used a wood job-made ladder (4 Ft Ladder) to gain access and as a means of egress in a trench that was 4 feet (1.22m) or more in-depth. The ladder did not extend at least 3 feet (.9 m) above the upper landing surface to which it was used to egress, and it was not secured at its top to a rigid structure, exposing employees to cave-in hazards.  NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.
