29 CFR 1926.451(c)(2)(i):Footings shall be level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement. a) On or about December 28, 2023, the employer failed to ensure that the footings of their 24' high scaffolding system was properly supported to prevent settling or displacement, in that the 2x4 (mudsills) on the northern end of the scaffolding were inadequately positions beneath the buck. As a result, employees were exposed to a 24' high fall hazard.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. a.) On or about December 28, 2023, the employer did not conduct adequate inspections of the scaffolding in that they failed to identify several visible defects including: inadequate supports of the scaffolding footings, absence of stacking pins in the frame of the scaffolding system, and missing top-rail and mid-rail components on the scaffolding frame. As a result employees were exposed to a fall hazard of 24' while working from the frame scaffolding.
