On or about December 20, 2023, at 285 Eyre Avenue, Merritt Island, Florida, 32953, employees were exposed to 8 feet -9-inch fall hazards while conducting roofing activities on a 2/12 pitched roof, in that, fall protection was not used.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a.) On or about December 20, 2023, at 285 Eyre Avenue, Merritt Island, FL, 32953, employees used a multi positioned ladder that was set approximately 12-inches above the roof landing to access the work area on the roof, thus exposing the employees to fall hazards of 8-feet 9-inches to the lower ground.
