29 CFR  1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about December 20, 2023, employees were exposed to a fall hazard while accessing an excavation using a ladder whose side rails did not extend at least 3 feet above the landing surface, was not secured at its top,  and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about December 20, 2023, employees were exposed to an engulfment hazard while working in an excavation with a depth exceeding more than 5 feet without adequate protection from cave-ins.   Blazey Construction Services, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1522716, citation number 2, item number 2, and was affirmed as a final order on June 04, 2021, with respect to a workplace located near the intersection of Cypresswood and James C Leo Drive, Spring, TX 77373.
