29 CFR  1910.106(d)(4)(iv):Each inside storage room was not provided with either a gravity or a mechanical exhaust ventilation system designed to provide for a complete change of air within the room at least six times per hour:  On or about December 21, 2023, the employer did not ensure that their flammable liquids storage room was equipped with a ventilation system that provided complete changes of air at least six times per hour.  The installed ventilation system did not work properly, exposing employees to injuries related to fire and explosion.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR  1910.178(q)(7):Industrial trucks were not examined before being placed in service:  On or about December 21, 2023, the employer did not ensure that powered industrial trucks were examined daily for conditions that could adversely affect the truck's safety before being placed into service.  Employees operated lift trucks to move various materials throughout the facility.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
