29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)  a) On or about 12/28/2023 and at times prior, at the worksite located at 12781 South U.S. Highway 41, Gibsonton, Florida 33534, the employer exposed employees to struck-by hazards, in that, the parking brake on the Blue Lull (Genie GTH-844) Powered Industrial Truck was found not operational during pre-use inspections and was not taken out of service.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) Throughout the North Bay: On or about 12/28/2023 and at times prior, at the worksite located at 12781 South U.S. Highway 41, Gibsonton, Florida 33534, the employer exposed employees to fire hazards, in that extension cords and/or GFCI extension cords were daisy chained together violating the listing and labeling of the equipment.  b) Throughout the South Bay: On or about 12/28/2023 and at times prior, at the worksite located at 12781 South U.S. Highway 41, Gibsonton, Florida 33534, the employer exposed employees to fire hazards, in that extension cords, GFCI extension cords, and/or power strips were daisy chained together violating the listing and labeling of the equipment.
