29 CFR  1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage, sharp corners and projections, or pinch points:  a)  Jobsite located at 5915 Germantown Avenue, Philadelphia, PA 19144 - On or about January 3, 2024, the employer did not ensure a flexible cable was protected from damage, sharp corners and projections, or pinch points. A flexible cable was installed in a circuit breaker panel on the 5th floor of the building and the flexible cable ran through doorways and pinch points without protection.

29 CFR  1926.502(b)(1): The top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  a) Jobsite located at 5915 Germantown Avenue, Philadelphia, PA 19144 - On or about January 3, 2024, the employer did not ensure the top edge height of the top rail was 42 inches, plus or minus 3 inches, above the walking/working level. An employee was engaged in electrical work near an elevator shaft that did not contain a top rail, thus exposing the employee to falls hazards of approximately 7 feet.
