A flexible cable was installed in a circuit breaker panel on the 5th floor of the building and the flexible cable ran through doorways and pinch points without protection.

An employee was engaged in electrical work near an elevator shaft that did not contain a top rail, thus exposing the employee to falls hazards of approximately 7 feet.
