29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:  location:  exterior of building, Prospect St. & Hawthorne Ave. on or about: 7/12/23  a) The employer did not ensure that all working levels of the tubular welded frame scaffold used by employees were fully planked. The employees were exposed to a fall of 60 feet.  location:  exterior of building, Prospect St. & Hawthorne Ave. on or about: 7/12/23  b) The employer did not ensure that all working levels of the tubular welded frame scaffold used by employees were fully planked.  Employees were working from the crossbraces. The employees were exposed to a fall of 60 feet.  location:  exterior of building, Prospect St. & Hawthorne Ave. on or about: 7/12/23  c) The employer did not ensure that all working levels of the tubular welded frame scaffold used by employees were fully planked.  Employees were working from the frames themselves.  The employees were exposed to a fall of 60 feet. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access:  location:  exterior of building, Prospect St. & Hawthorne Ave. on or about: 7/12/23  a) Employees were working on a tubular welded frame scaffold. The employees were not provided with adequate access. The employees were climbing the walk through frames and crossbraces.  NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  location: Northeast corner of building 0n or about: 7/12/23  a) An employee, who was working at the edge of the building, was not provided with adequate fall protection. The midrail was missing. The employee was exposed to a fall of 60 feet.  NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
