This exposed employees to fall hazards greater than six (6) feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about January 4, 2024, employees used portable ladders to access a roof and the side rails did not extend at least 3 feet above the landing surface. This exposed employees to a fall hazard to the ground below. Instance a: A portable ladder on the west side of the building against the north roof eve. Instance b: A portable ladder on the west side of the building against the east roof eve.
