29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about January 3, 2024, hazardous energy control procedures were not utilized on the Toyoda CNC Model Stealth AA 1165 during setup activities such as tool changeouts, fixture changeouts, or pallet changeouts. b) On or about January 3, 2024, hazardous energy control procedures were not utilized on the Haas CNC Model DT during setup activities such as tool changeouts, fixture changeouts, or pallet changeouts. c) On or about January 5, 2024, and times prior, hazardous energy control procedures were not utilized on the Haas CNC Model YT during setup activities such as tool changeouts, fixture changeouts, or pallet changeouts. d) On or about December 29, 2024, hazardous energy control procedures were not utilized for CNC Mori Seiki Model NH4000 during setup activities such as tool changeouts, fixture changeouts, or pallet changeouts. e) On or about January 5, 2024, and times prior, hazardous energy control procedures were not utilized on the Makino CNC Machine Model a51nX during setup activities such as tool changeouts, fixture changeouts, or pallet changeouts. f) On or about January 5, 2024, and times prior, hazardous energy control procedures were not utilized on the Toyoda CNC Machine Model FH500J during setup activities such as tool changeouts, fixture changeouts, or pallet changeouts.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about December 29, 2023, the Mori Seiki NH4000 Machine was/were not equipped with one or more methods of machine guarding to prevent the operator form accessing rotating parts, and/or the point of operation inside the machines during operation. b) On or about May 9, 2024, one or more methods of machine guarding were not provided to protect operators from accessing rotating parts, and/or the point of operation during minor servicing and maintenance activities such as coolant hose adjustments on the Mori Seiki CNC Milling Machine model NH4000.
