State Route 18, Lot 91 Fostoria OH 44830, did not have a plan or a program to identify job site hazards which assists employees in maintaining a safe working environment and accident prevention. Such programs would support the employer and employees in identifying and avoiding potential hazards such as struck-by and crushed-by hazards during job site operations, decreasing the probability of employee injury or illness.

State Route 18, Lot 91 Fostoria OH 44830, did not ensure that employees properly blocked or cribbed the base of the Pittsburgh Brand 20 ton air/hydraulic jacks, and ensured that a wood block was placed between the jack cap and the steel frame of the mobile home to prevent slippage of the load. Employees performing mobile home teardown and transportation preparation activities were exposed to struck-by and crush-by hazards while jacking/lifting a 1994 Fleetwood mobile home (S/N TNFLR26A38998RL).

Employees performing mobile home teardown and transportation preparation activities were exposed to struck-by and crush-by hazards while jacking/lifting a 1994 Fleetwood mobile home (S/N TNFLR26A38998RL).
