29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  On or about January 8, 2024 the employer K&M Transport LLC, at jobsite located at 8702 W. State Route 18, Lot 91 Fostoria OH 44830, did not have a plan or a program to identify job site hazards which assists employees in maintaining a safe working environment and accident prevention. Such programs would support the employer and employees in identifying and avoiding potential hazards such as struck-by and crushed-by hazards during job site operations, decreasing the probability of employee injury or illness.

State Route 18, Lot 91 Fostoria OH 44830, did not ensure that employees were instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to the work environment and to control or eliminate any hazards or exposures to injury. Employees performing mobile home teardown and transportation preparation activities were exposed to struck-by and crush-by hazards while jacking/lifting a 1994 Fleetwood mobile home (S/N TNFLR26A38998RL).

29 CFR  1926.305(c):A wood block was not placed between the cap and the load where there was a possibility of slippage of the metal cap of the jack:  On or about January 8, 2024 the employer K&M Transport LLC, at jobsite located at 8702 W. State Route 18, Lot 91 Fostoria OH 44830, did not ensure that employees properly blocked or cribbed the base of the Pittsburgh Brand 20 ton air/hydraulic jacks, and ensured that a wood block was placed between the jack cap and the steel frame of the mobile home to prevent slippage of the load. Employees performing mobile home teardown and transportation preparation activities were exposed to struck-by and crush-by hazards while jacking/lifting a 1994 Fleetwood mobile home (S/N TNFLR26A38998RL).

State Route 18, Lot 91 Fostoria OH 44830, did not ensure that that the load of the lifted structure was sufficiently supported by jack stands, cribbing or blocking to prevent collapse due to shifting, jack movement, and/or jack failure. Employees performing mobile home teardown and transportation preparation activities were exposed to struck-by and crush-by hazards while jacking/lifting a 1994 Fleetwood mobile home (S/N TNFLR26A38998RL).
