29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  At the Topping Line, employee(s) engaged in production and maintenance activities were exposed to amputation and laceration hazards in that a projecting end shaft from the cheese machine conveyor belt projected more than one half the diameter of the shaft. The diameter of the horizontal shaft was approximately 1 1/4 inch. The horizontal shaft projected approximately 1 inch.

29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  At the Main Facility, employee(s) engaged in food processing and maintenance operations were exposed to laceration and amputation hazards. The employer did not provide a written certification that a workplace hazard assessment had been performed whereby employee(s) were working in and around machinery with mandatory PPE (gloves) which can create a greater hazard when working around food processing machinery. The employer should have identified the greater hazard on the written hazard assessment certification. This resulted in an employee who suffered a laceration resulting in hospitalization for fractured arm bones after attempting to clear a clog on a machine. The fingertip of the employee(s) glove was pulled into the machine's sprockets and chains which pulled the employee(s) arm into the machine resulting in injury.
