On or about January 6, 2024, at 93 Caribbean Road, Naples, FL 34112, the employer did not ensure the fall protection system, which the rope was frayed, was inspected before each use while engaged in tree trimming/removal, exposing employees to fall hazards approximately 50ft above ground.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  On or about January 6, 2024, at 93 Caribbean Road, Naples, FL 34112, employees engaged in tree trimming/removal were not trained in the safe use of personal fall protection systems and aluminum extension ladders, exposing them to fall hazards approximately 50ft above ground.
