On or about January 9, 2024, in the facility, employees were exposed to unsanitary conditions as well as slip, trip and fall hazards when the mezzanine and staircase area(s) contained garbage, sticky residue, wood and unknown substances throughout the walking and working surface(s).

On or about January 9, 2024, on the mezzanine, employees were exposed to trip and slip hazards when the walking/working surface had a rectangular hole in the floor measuring thirteen inches (13") by fourteen inches (14").    b.  On or about January 9, 2024, on the mezzanine, employees were exposed to trip and slip hazards when the walking/working surface had a round hole in the floor measuring  ten and one half inches (10.5") in diameter.
