Instance 2- the Nissan Forklift was not inspected daily before put into service at Houston Metal Sawing, LLC.

On or about January 12, 2024, employees were exposed to carbon monoxide poisoning when a propane powered forklift was being used inside the shop with no ventilation, or open doors.

Instance 4- In the machine shop, employees were exposed to struck-by hazards while using a 560 Velox- 1 Bandsaw that did not have a point of operation guard on it.
