29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates and mud sills or other adequate firm foundation: a) Jobsite located at 615 East Mount Pleasant Avenue, Philadelphia, PA 19119 - On or about January 12, 2024, the employer did not ensure scaffold supports were on base plates and mud sills, thereby exposing employees conducting masonry activities on the scaffold to fall hazards of approximately 11.5 feet from unstable scaffold.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Jobsite at 615 East Mount Pleasant Avenue, Philadelphia, PA 19119 - On or about January 12, 2024, the employer failed to ensure an employee performing masonry activities was protected from falls by the use of a fall protection system. An employee worked on a tubular welded scaffold to while performing masonry activities without fall protection, thereby exposing the employee to falls of approximately 11.5 feet.
