29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units: (a): Exterior Scaffold - Center Ave - Unstable objects were used to support a fabricated frame scaffold. Employees were exposed to fall hazards from potential scaffold collapse when working on the fabricated frame scaffold that was not properly supported. The scaffold was supported by hollow masonry blocks filled with sand, on or about January 12, 2024. There is no abatement certification or documentation required for this item. (b): Exterior Scaffold - Stevens Ave - Unstable objects were used to support a fabricated frame scaffold. Employees were exposed to fall hazards from potential scaffold collapse when working on the fabricated frame scaffold that was not properly supported. The scaffold was supported by hollow masonry blocks filled with sand, on or about February 20, 2024. There is no abatement certification or documentation required for this item. (c): Exterior Scaffold - Stevens Ave - Unstable objects were used to support a fabricated frame scaffold. Employees were exposed to fall hazards from potential scaffold collapse when working on the fabricated frame scaffold that was not properly supported. The top four scaffold levels were supported with the legs placed on the planks of the first frame, on or about February 20, 2024. 29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a): Exterior Scaffold - Center Ave - Employees were exposed to fall hazards of approximately 20 feet when working on a scaffold without proper fall protection. The employees were not utilizing personal fall arrest systems and the scaffold was not equipped with a proper guardrail system, on or about January 12, 2024. There is no abatement certification or documentation required for this item. (b): Exterior Scaffolds - Stevens Ave - Employees were exposed to fall hazards of approximately 30 feet when working on a scaffold without proper fall protection. The employees were not utilizing personal fall arrest systems and the scaffold was not equipped with a proper guardrail system, on or about February 20, 2024. Crossbraces were used as a means of access. (a): Exterior Scaffold - Center Ave - Employees were not provided with proper access to the scaffold. Employees were climbing the scaffold frames and cross braces for access , on or about January 12, 2024. There is no abatement certification or documentation required for this item.
