(a) At 2581 Davisville Road North Kingstown, Rhode Island 02852 (Pier #1) dockside: On or about 01-15-2024, employees were exposed to slip, trip, and fall hazards, while performing line handling duties on the untreated icy ground surface conditions.

(a) At 2581 Davisville Road North Kingstown, Rhode Island 02852 (Pier #1) dockside: On or about 01-15-2024, employees were exposed to vehicular events, to include struck-by and struck-against hazards, while operating and working near van #4, which had broken rearview mirrors and a transmission shifter that was not maintained in a safe working condition.
