29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  At the workplace located at 3725 West Harry Street, Wichita, Kansas 67213, employee(s) engaged in forklift, or powered industrial truck (PIT), operations, handling liquified petroleum gas (LPG) cylinders at the warehouse near the loading dock, were exposed to projectile hazards. Liquified petroleum gas (LPG) cylinders, that were not in use, were not secured to protect against abrasion or strikes by heavy machinery. The employer stored liquified petroleum gas (LPG) cylinders in a location where they were exposed to forklift and vehicular traffic near the loading dock. A Yale forklift (model: GLP050RGNUAE090) enters and exit the location regularly. [See: Compressed Gas Association (CGA) Pamphlet P-1-1965: Section 3.3.8]

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  At the workplace located at 3725 West Harry Street, Wichita, Kansas 67213, employees were exposed to struck-by hazards. The employer did not ensure that employees operating powered industrial trucks, such as the Yale forklift (model: GLP050RGNUAE090), were provided with powered industrial truck training to verify that all operators were provided with the necessary education to operate the equipment safely.

A Yale forklift (model: GLP050RGNUAE090) in the warehouse had inoperable lights.
