29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) 227 Pennsylvania Avenue, Centre Hall, PA, Side Roof,  - Employees performed residential roofing activities on a two-story residential roof,  approximately 20 feet above grade with an 8:12 roof pitch, without utilizing any means of fall protection, on or about January 18, 2024.  (b)  227 Pennsylvania Avenue, Centre Hall, PA, Back Roof,  - Employees performed residential roofing activities on a two-story residential roof,  approximately 20 feet above grade with an 6:12 roof pitch, without utilizing any means of fall protection, on or about January 18, 2024.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 227 Pennsylvania Avenue, Centre Hall, PA, Garage Roof - Employees were performing residential roofing operations on a two story residential roof,  the portable ladder did not extend 3 feet above the upper landing, on or about January 18, 2024.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
