29 CFR  1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):  On or about December 20, 2023 at Westwood Mills Corp. on 55 Sharp St, Hingham, MA 02043, an employee was exposed to an 8-hour time-weighted average carbon monoxide (CO) level of 254.7 ppm, approximately five times the Permissible Exposure Limit (PEL) of 50 ppm, while operating a CAT propane-powered forklift (Model: GC30; SN: 6EM02491). Feasible administrative or engineering controls were not implemented to reduce carbon monoxide levels in the workplace. Feasible methods administrative or engineering controls of control included but were not limited to: 1. Increased ventilation in the work space. 2. Installing meters & alarms to detect elevated levels of carbon monoxide 3. Switching to an alternative fuel to power the forklifts. 4. Ensuring routine maintenance of the forklifts that included exhaust air quality testing
