29 CFR  1910.178(l)(6):Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  On or about January 23, 2024, at the establishment located at 101 East Chestnut Street Chilton, Wisconsin; employer did not have certification documentation of forklift training and evaluation for employees who operate forklift trucks.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about January 23, 2024, at the establishment located at 101 East Chestnut Street Chilton, Wisconsin; the point of operation on the Hanna Hydraulic Cylinder tube bender was not guarded to prevent employees' fingers from reaching the pinch point created by the die and piece of aluminum being bent.
