29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval:  (a) Jobsite: 6610 Hasbrook Avenue, Philadelphia, PA 19111 - On or about January 23 2024,The employer did not ensure employees who were utilizing a job-made platform that was on the forks of a Toyota forklift was designed, and approved by the manufacturer; thus exposing employees to fall hazards of approximately 12 feet from the ground level.
