29 CFR 1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials: (a) Block 24 Hawthorne Drive, Harrisburg PA - On or about January 23, 2024, an employee conducted roofing activities approximately 12 feet above grade on a roof with a slope of 6-12. The employee used a personal fall arrest system with a positioning device that were not constructed of drop forged steel, exposing the employee to a fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(21):Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) Block 24 Hawthorne Drive, Harrisburg PA - On or about January 23, 2024, an employee conducted roofing activities approximately 12 feet above grade on a roof with a slope of 6-12. An employee to use a personal fall arrest system that had worn or damaged nylon webbing and also corrosion damage on the dee ring and buckles. (b) Block 24 Hawthorne Drive, Harrisburg PA -On or about January 23, 2024, an employee conducted roofing activities approximately 12 feet above grade on a roof with a slope of 6-12. An employee to use a personal fall arrest system with a 4-foot shock absorbing lanyard which was shortened using duct tape. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
