OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to potential crushing hazards from falling loads:  a) Left side of Inspection Bay, Tim Voorhees Auto Repair, Inc., 501 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a two-post surface mounted Ben Pearson Tubemaster Corp., Lift Model Number 9000-P, Serial Number 01970888, was operated without being inspected and certified since installation approximately 25 years ago.   b) Right side of Inspection Bay, Tim Voorhees Auto Repair, Inc., 501 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a two-post surface mounted Ben Pearson Tubemaster Corp., Lift Model Number 9000-P, Serial Number 04971134, was operated without being inspected and certified since installation approximately 25 years ago.   c.) Left side of Mechanical Bay, Tim Voorhees Auto Repair, Inc., 501 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a two-post surface mounted Rotary Lift, Lift Model Number SPOA9-200, Serial Number AFM98D115, was operated without being inspected and certified since installation approximately 15 years ago.   d.) Right side of Mechanical Bay, Tim Voorhees Auto Repair, Inc., 501 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a two-post surface mounted Ammco Lift, Lift Model Number S290LR1, Serial Number T9-18477, was operated without being inspected and certified since installation approximately 15 years ago.   e.) Left side of Engine Repair Building, Tim Voorhees Auto Repair, Inc., 511 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a two-post surface mounted A Plus Lift, Lift Model Number HW-10KBP-A, Serial Number 7619B3941, was operated without being inspected and certified since self-installation approximately 6 months ago.   f.) Alignment Bay- Engine Repair Building, Tim Voorhees Auto Repair, Inc., 511 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a four-post surface mounted Bend Pak Lift, 14,000 pound capacity lift, was operated without being inspected and certified since installation approximately 2 years ago.   g.) Right side of Engine Repair Building, Tim Voorhees Auto Repair, Inc., 511 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, a two-post surface mounted Rotary Lift, (model and capacity not listed on equipment), was operated without being inspected and certified since installation approximately 2 years ago.   Abatement certification is required.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) 501 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, Employees are exposed to corrosive chemicals including, but not limited to, Purple Power Industrial Strength Cleaner/ Degreaser (pH: 12.98), Castle Endura Heavy Penetrating Grease which causes serious eye irritation, Mineral Spirits which causes eye irritation, Prema PBS-32 Bead Sealer which causes eye irritation, and Castle 2-22 d-limonene Hard Surfaces Cleaner which is irritating to eyes, and are not provided with an eyewash for immediate emergency use in their work area.  b) 511 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, Employees are exposed to corrosive chemicals including, but not limited to, Purple Power Industrial Strength Cleaner/ Degreaser (pH: 12.98), Castle Endura Heavy Penetrating Grease which causes serious eye irritation, and Mineral Spirits which causes eye irritation, and are not provided with an eyewash for immediate emergency use in their work area. 29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:  a) Electrical Panel, Inspection Bay, Tim Voorhees Auto Repair, Inc., 501 Day Hollow Road, Endicott, NY 13760, on or about 1/25/2024, Tire Mounting equipment and tires were stored directly in front of the electrical panel servicing the building.   Abatement certification is required.
