29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:  On or about January 25th, 2024, employees engaged in maintenance related operations located 4880 South 134th Street, Omaha, Nebraska 68137, were exposed to energy source releases such as electrical and thermal hazards in that the employer developed one set of procedures for the following injection molding press #'s 40, 41, 100, 106, 108, 109, 111, 160, 164, 199, 200, 201, 207, 218, 300, and 401 without addressing their differences.  Instances of failure include but are not limited to:   1. Different manufacturers have different isolation procedures and locations for LOTO points.  2. The electrical/Amperage energy sources are not the same and vary from press to press.   3. The current LOTO procedure does not state how or where to turn off heaters. 4. The current LOTO procedure does not state how or where to shut off water. 5. The current LOTO procedure does not state how or where to disconnect air lines for mold blow off.  6. The Current LOTO procedure does not state how to test  the Master power and Operational power switches to verify the energy sources have been isolated or eliminated.
