On or about January 26, 2024, the employer had junction boxes and breaker panels with missing knockouts and blanks in several locations, exposing employees to electrical hazards. A. Northwest side of the main shop. B. Wall is in the hot work area on the southeast side of the shop.

29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(es) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals. On or about January 26, 2024, an employee performed machine tasks using a damaged treadle/pedal. The pedal did not have a cover that would prevent accidental activation.
