29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8-hour Time Weighted Average concentration of 50 ppm:   On or about January 29, 2024, and at times prior thereto, employees in the bakery areas were exposed to carbon monoxide exceeding the permissible exposure limit (PEL) of 50 parts per million (ppm) in the following instances (a-k):  a. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 186 parts per million (ppm) over an 8-hour time-weighted average (TWA), which is approximately four (4) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  b. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 153 parts per million (ppm) over an 8-hour time-weighted average (TWA), which is approximately three (3) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  c. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 328 parts per million (ppm) over an 8-hour time-weighted average (TWA), which is approximately seven (7) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  d. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 102 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately two (2) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  e. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 184 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately four (4) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 180-minute period, with zero valve exposure assumed during a 300-minute non-sampled period.  f. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 93 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately two (2) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  g. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 159 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately three (3) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  h. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 334 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately seven (7) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  i. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 221 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately four (4) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  j. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 103 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately two (2) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.  k. An employee in the bakery areas was exposed to carbon monoxide at a concentration of approximately 225 parts per million (ppm) over an 8-hour time-weighted average (TWA), is approximately five (5) times the permissible exposure limit (PEL). The carbon monoxide exposure was based on post-exposure carboxyhemoglobin (COHb) measurements over a 330-minute period, with zero valve exposure assumed during a 150-minute non-sampled period.
