Employees were exposed to potential eye injuries from splashing hazardous chemicals such as but not limited to PT(Water Based Ink Dissolver). 29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. (a) On or about January 19, 2024 the employer did not ensure that employees who perform service and maintenance tasks on the Totani Pouch Machine were trained on the means and methods to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up, energization and/or release of stored energy which could result in caught-in, crushing and amputation hazards from the heat stomp or in-going nip points created by the rollers when feeding the zipper through the zipper seal station. (b) On or about January 30, 2024, the employer did not ensure that employees who perform service and maintenance tasks on the Totani Pouch Machine were trained on the means and methods to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up, energization and/or release of stored energy which could result in caught-on, crushing and amputation hazards from the heat stomp or in-going nip points created by the rollers while performing set up tasks for production. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about January 19, 2024, the employer did not ensure that employees implemented energy control application steps prior to allowing them to set up the Totani Pouch Machine by feeding the zipper through the zipper seal station. The machine was not shut off from the digital panel, nor was the control panel de-energized and locked out to prevent the unexpected start-up during this servicing and maintenance activity, exposing them to crushing and amputation hazards from the unexpected movement of the heat stomp. (b) On or about January 30, 2024, the employer did not ensure that employees implemented energy control application steps prior to allowing them to set up the Totani Pouch Machine for production. The machine was shut off from the digital panel, but not de-energized and locked out to prevent the unexpected start-up during this servicing and maintenance activity, exposing them to crushing and amputation hazards from the unexpected movement of the heat stomp and in-going nip points created by rollers. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about January 19, 2024 the employer did not ensure Totani Pouch Machine was guarded to prevent access to hazardous locations along the line while in operation. Employees were exposed to crushing injuries and amputations created by the heat stomp and caught-in hazards created by in-going nip points from rollers. The equipment had been installed with light curtains, but they were not functioning for over a year. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Eagle Flexible Packaging, Inc- worksite at 1100 Kingsland Drive in Batavia, IL: on or about January 30, 2024, the employer failed to provide within four business hours the requested OSHA 300 Log of Work-Related Injuries and Illnesses for 2024 year-to-date, and OSHA 301 Injury and Illness Incident Report(s) for all recordable injuries in 2024 year-to-date. There is no abatement certification or documentation required for this item.
