All functional operating mechanisms for maladjustment interfering with proper operation were not inspected daily, including observation during operation for any defects which might appear between regular inspections, and/or all deficiencies such as listed were not carefully examined and determination made as to whether they constitute a safety hazard: a) On or about January 31, 2024, the employer did not ensure the daily inspection of the (Advantage North) 5-ton overhead crane included examination of the rusted structural members and a determination of the safety hazard. Employees were exposed to struck-by hazards from the overhead crane and the supporting structure. b) On or about January 31, 2024, the employer did not ensure the daily inspection of the (Advantage South) 5-ton overhead crane included examination of the rusted structural members and a determination of the safety hazard. Employees were exposed to struck-by hazards from the overhead crane and the supporting structure. c) On or about January 31, 2024, the employer did not ensure the daily inspection of the (Edge North) 5-ton overhead crane included examination of the rusted structural members and a determination of the safety hazard. Employees were exposed to struck-by hazards from the overhead crane and the supporting structure. d) On or about January 31, 2024, the employer did not ensure the daily inspection of the (Edge South) 5-ton overhead crane included examination of the rusted structural members and a determination of the safety hazard.
