On and before January 31, 2024, the employer did not protect employees from hazards by ingoing nip points. On January 18, 2024, one employee was injured after being directed to use the CRS #11 Table Bender without the use of machine guards or other means to keep workers away from the point of operation. This condition exposed employees to crushing hazards.

On and before January 31, 2024, the employer used flexible cords and/or cables as a substitute for the fixed wiring of a structure to energize the the CRS #11 Table Bender, exposing employees to electric shock and tripping hazards.
