29 CFR 1910.106(b)(6): "Sources of ignition." In locations where flammable vapors may be present, precautions shall be taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat. On or about January 30, 2024, employees cleaning the tunnel walls with a gasoline fueled pressure washer were exposed to fire and inhalation hazards; the employer did not take precautions to prevent the ignition of flammable vapors from unleaded gasoline by eliminating or controlling the sources of ignition to include but not limited to the hot surfaces of the pressure washer muffler.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) On or about January 30, 2024, employees were working in the tunnel area directly next to the car wash tunnel equipment. The employer did not develop or document procedures for the control of pneumatic and electrical energy for employees engaged in tightening the tunnel roller and chain conveyor belt, and conducting maintenance or cleaning the tunnel equipment inside the tunnel area.
