29 CFR  1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.  a) On or about 2/1/2024, employer did not ensure that employees retrieving materials from the mezzanine located across from the glue line had exit routes that were free and unobstructed in that there were tables and palletized product  18 inches directly in front of the stairs (exit route) to the mezzanine.

(a) On or about 2/1/2024, employer did not ensure that a fire exit in the production area used by employees was free from flammable liquid storage such as but not limited to two 55 gallon drums with spray adhesive( Akron Paint and Varnish).   (b) On or about 2/1/2024, employer did not ensure that a fire exit in the production area used by employees was free from stored flammable liquids such as but not limited to five 33.5lbs propane cylinders.

29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  a) On or about 2/1/2024, employer did not ensure that employees that were operating forklifts such as the Yale 155U (SN:E187V04155U) and the Toyota 5EGCU25 (SN:70458) had a refresher evaluation at least once every three years. Last date evaluated was 05/05/2015.
