29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  On or about February 2, 2024, the employer did not ensure that extension cords located at the Automatic Line were inspected prior to use. The concrete mixture contained several products that were labelled as corrosives and respirable hazards and identified as being able to cause serious chemical burns and eye damage, including products such as white portland cement type I-II, portland cement, Arcosa Lightweight--Expanded Shale Lightweight Aggregate, ConAir, and NiroCast K which contain products including but also not limited to calcium oxide, crystalline silica, sulfonic acids C14-16-alkane hydroxy C14-16 alkene sodium salts, sodium hydroxide, nitrocalcite tetrahydrate, and sodium rhodanate.

On or about February 2, 2024, the employer failed to develop and maintain product identifiers or information labels for secondary containers such as spray bottles used to spray a mixture of products onto the molds of the automatic line. The mixture includes but is not limited to NitroCast K and ConAir that contain chemical ingredients including but not limited to sulfonic acids C14-16-alkane hydroxy C14-16 alkene sodium salts, sodium hydroxide, nitrocalcite tetrahydrate, and sodium rhodanate. Without proper labelling, employees may unintentionally expose themselves to these products that can cause serious injuries including eye injuries and chemical burns.

Chemical-specific information must always be available through labels and safety data sheets.  On or about February 2, 2024, the employer failed to train employees on the company's Hazard Communication (HAZCOM) program and the different chemical hazards found in their workplace.
