29 CFR  1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  a. On or about December 4, 2023, at the worksite located at 222 West Lane Ave, Columbus, OH 43201, an employee suffered an injury, that resulted in days away from work, after falling from the bed of a truck while rigging lumber to be unloaded by a tower crane. The employer did not ensure that the injury was recorded on the OSHA injury and illness forms.
