29 CFR 1910.22(a)(1): 29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about January 30, 2024, and times prior thereto at this workplace, employees conducted grain handling operations, the employer did not keep a clean and orderly workplace by not disposing of out of service grain handling equipment such as but not limited to fans, motors, cowls, metal housing placed along side of the grain bins near grain bin #5. The out of service grain handling equipment creates refuge cover for vermin, snakes and poisonous insects.

On or about January 30, 2024, and times prior thereto at this workplace, employees conducted grain handling operations, in grain bin (silo) #7 while the grain sweep auger was energized and was not disconnected / locked out and tagged out. Employees were exposed to caught-in hazards resulting in amputations or death.
